Why would I complete this Application Form?

I need to register and I would like to do so by post (paper form) and not online.

I will receive a letter telling me that my application form has reached the Electoral Registration Office at the Council. 

 

When my form arrives at the Electoral Registration Office my details are added to the Register Database.  Then my Name, Date of birth and National Insurance details are checked against government digital portals. If my details match then I am added to the Electoral Register.  I will then receive a letter to confirm I have been added to the register.

 

If my details to do not match information on the government portals then I will receive a letter with further instructions.

Invitation To Register (ITR) Form (English & Welsh)

 

I would like to vote by post and not go to a polling station on poll day.

My ballot papers are delivered to my home address or to any designated forwarding address. My ballot papers will be delivered to me as part of a pack which I will need to complete and return in order for my vote to be counted. I should receive these within 10 days of poll day. I must provide my signature and my date of birth as this information is used to verify my ballot papers.

I would like someone else to vote on my behalf at the polling station.

I will receive a letter confirming that the person I have nominated to cast my vote for me (my proxy) has been approved.  My proxy will receive a polling card before poll day that has my name on it.  My proxy must visit the polling station in person, on poll day, with the poll card and cast my vote for me.

 

*Please note that the person you nominate must be on the Electoral Register or they can not be your proxy*

I will receive a letter confirming that my record on the Electoral Register has been amended and I am now opted out.  I do not need to do anything else unless I change address.  If I move house I must opt-out again from my new address.

Opt Out Form - English & Welsh

I need to change my name on the Electoral Register.          

 *Don't forget - you need to send evidence that you have changed your name with this form!  This can be a marriage certificate, amended birth certificate or a deed poll letter/certificate*

 

The details on my form will be checked against the details on my evidence.  If they match then my name will be changed on the register.  I will then get an amendment letter saying my name has been changed.

 

If the details do not match then I will receive a letter with further instructions.

Types of Waiver Form

I want to vote by post but cannot provide a signature.        

*This form allows you to vote without having to provide a signature. There must be a valid reason, such as an inability which may mean that your signature is inconsistent. If you apply for a postal waiver you will only have to provide your date of birth when completing your postal application and postal ballot paper.*
