Mental Health Service
Mental illness affects 1 in 5 people in the UK during their
lifetime. We offer local services to help people who suffer from
mental distress and who live in the Vale of Glamorgan.
The Community Mental Health Team is made up of a Team Manager
and 12 Social Workers. We also have a Community Support Work
Manager and a team of Community Support Workers.
What do we do
We accept referrals and liaise with people from a wide variety
of backgrounds including:
- Self-referral, carers, family, friends and neighbours
- Health professionals including GPs, Psychiatrists,
psychologists, community psychiatric nurses, and hospital
staff
- Voluntary agencies
- Other departments within the Authority, including Housing and
Childrens Services
- Probation, police, courts and solicitors
Community Care
The Community Care Act requires Local Government to carry out
assessments of peoples needs and where possible provide services to
those individuals who qualify. The services defined in a care plan
may include advice, support, domiciliary care, residential care or
nursing care.
Hospital care
If you receive hospital care, our Social Work service will
provide support and advice and help you to keep in contact with
your family, friends and the community in general. Together with
other professionals, Social Workers have an important role in
planning for hospital discharges and providing follow-up care.
