- The Health, Social Care and Wellbeing Strategy is based on the needs of the local population within the Vale of Glamorgan.
- It has been produced as part of a community partnership including the public and private sector and will be the basis of our joint working over the next few years.
By working together, the strategy will benefit the whole population by:
- Helping people remain healthy for as long as possible.
- Ensure that when people do become unwell, or need support, that they receive timely and appropriate care.
Working as a partnership enables us to pay more attention to the wider determinants of health – Heredity (age and sex); Environment (natural and manmade; social: employment, education, housing): Lifestyle (smoking, exercise, drugs): Health and Social Care Services.
The strategy looks at all of the following through the National Service Framework:
Some of our ideas for addressing the needs of our key priority groups include:
- Improving the lifestyles of young people by lowering the levels of childhood obesity, smoking and binge drinking
- Insuring services for people with learning disabilities and their carers are well planned
- Providing the majority of care for people who need mental health support services in the community
- Improving cancer services to put cancer survival rates in the Vale of Glamorgan in line with the top 25 per cent
- Simplifying access to abroad range of home and/or community based services for people with chronic conditions
- Developing an ‘extracare’ housing scheme that provides services for older people with arrange of social and health care needs at home
A more detailed implementation plan along with a resource framework and a performance management framework are currently being developed to ensure the delivery of the Strategy. These will be publicised in due course.
