Regional Collaboration Fund End of Programme Report - Remodelling Social Care and Integration with Health


Purpose of the Report

  1. To provide Scrutiny Members with an update on the work that has been carried out over the past three years to remodel social care and integrate services with health using the Regional Collaboration Fund (RCF).

Reason for the Recommendations

1&2    To provide Scrutiny with an update on the how the Regional Collaboration Fund has been used to contribute to remodelling Social Care and integrating services with health across the Cardiff and Vale of Glamorgan region.


  1. In 2013/14, through the Regional Collaboration Fund, the Welsh Government approved funding to the Vale of Glamorgan and Cardiff Councils to establish collaborative and innovative projects which would assist with the development of remodelled adult community health and social care services. The programme was intended to accelerate the pace of change and deliver improvements for people using services across Cardiff and the Vale of Glamorgan. The Programme was established on a three years basis with indicative funding for years 2 and 3 subject to the budget being available.
  2. The following criteria were used in deciding which projects would be included within the Programme:
  • Be collaborative - all projects should include at least two partner organisations.
  • Result in integrated working - be that between the two councils or one/both councils with the Health Board.
  • Result in people-focused outcomes and benefits - for example, enhancing well-being, health, independence and do so more efficiently and effectively.
  • Generate demonstrable benefits for partner organisations - for example, reduced costs, improved performance indicators, enhanced staff development opportunities or increased operational efficiency.
  1. The Vale of Glamorgan Council managed the overall administrative arrangements for the RCF grant for the three years of the Programme. It has worked successfully with the City of Cardiff Council, the University Health Board (UHB) and third sector organisations on a number of projects that have delivered the required outcomes for the RCF.
  2. A Programme Manager was appointed at the end of May 2014 to establish programme/project management arrangements to oversee and monitor progress. Appendix 1 sets out the Programme Governance structure. The Programme focused initially on six projects:
  • Effective Community Resource Teams
  • Enhanced services for people with learning disabilities
  • Enhanced Occupational Therapy Services
  • Assistive Technologies
  • Streamlined Integrated Assessment
  • Improved Commissioning.
  1. The Programme Manager initiated an evaluation of the year 1 spend and benefit realisation before the budget for the second year of the programme was allocated. This ensured that the funding was spent on those projects that realised the greatest benefits and delivered the best outcomes for citizens.

Relevant Issues and Options

  1. Further detail on the evaluation of projects delivered in year 1 of the Programme is contained in Appendix 2. The evaluation resulted in the Board agreeing that the Programme continued to focus on four of the six projects. The remaining two projects became either 'business as usual' (Improved Commissioning) or delivered though the Intermediate Care Fund programme (Assistive Technologies).
  2.  Appendix 3 contains the 2014/15 end of year summary report setting out progress made for each of the projects and highlighting the benefits realised. Case studies were included to provide evidence of the difference these projects were making to the lives of people who use the services.
  3. The Regional Collaboration funding was reduced in 2015/16 and so the projects were realigned - building on the lessons learned from years one and two, to ensure maximum benefit was delivered with the resources allocated. The Programme focused on two projects, Enhanced Reablement Services and Enhanced Services for People with Learning Disabilities. The RCF Grant proved invaluable in enabling the Region to pilot new models of working that bring together health, social care and housing resources. Lessons learned are already being put into practice to introduce new delivery models which will help organisations meet the new requirements of the Social Services and Well-being (Wales) Act.
  4. A final progress report, set out in Appendix 4, was produced in May 2016. The last two projects have now become business as usual, with additional support provided through the Intermediate Care Fund. The services will continue to be monitored through the new Regional Partnership Board and Appendix 5 sets out the new governance arrangements which will oversee continued integration of health and social care services where this is the best way of achieving improved outcomes for residents.
  5. All six projects have worked towards and contributed to delivering the following outcomes:
  • Achieved greater integration of social care services with health.
  • Delivered a redesigned service that better meets the needs of service users.
  • Created more joint working between the Vale of Glamorgan and Cardiff Councils and the University Health Board.
  • Ensured citizens are able to retain independence for as long as possible while living more fulfilled lives.
  • Developed improved processes which have led to more efficient service provision.
  • Managed demand for services in a more focused, targeted way.
  • Introduced greater co-production in the design of services with effective partnership working to increase choice and control for people who use services, carers and citizens.
  1. In addition to improved outcomes for service users, processes have become more efficient and new models of working developed which have contributed to savings and cost avoidance. These savings are set out in appendices 3 and 4.

Effective delivery of the projects is intended to ensure that the Council's assets are managed efficiently and effectively and that best use is made of the resources available for social care services, establishing sustainable models of assistance and support as well as contributing to the most appropriate ways of meeting the needs of vulnerable people.
