Additional Learning Needs Transport Support - Request

Use this form to apply for transport for a child with additional learning needs

Please note that you must provide medical evidence to help us assess your request fairly. If you do not upload medical evidence you will not be able to submit your request.

To apply for school transport, you will need to provide medical evidence that clearly explains your child's needs. This must include:

A letter from a hospital consultant or specialist (not a GP) that explains:

• why your child cannot travel safely, even when accompanied by an appropriate person
• how long your child is likely to need transport support
• whether the condition is likely to improve
• any special transport arrangements or equipment needed
• when the condition will be reviewed

To keep receiving support, you will need to send updated medical evidence every year.
