Vale Community Resource Service (VCRS)
VCRS is a Therapy, Reablement Care and Support integrated service run in conjunction with Cardiff & Vale University Health Board and Vale of Glamorgan Council.
Within VCRS there is a team consisting of Health & Social Care Multi-Disciplinary Professionals who provide rehabilitation, reablement and recovery therapy to you in your home either when you come home from hospital or following a community referral.
We aim to help you be as independent as possible and to prevent any avoidable admission to hospital or care home and if needed to assist you with future care and support needs.
VCRS
What is VCRS and where are we based?
We are community service team with therapy, health and medical staff employed by Cardiff and Vale University Health Board and social care staff employed by the Vale of Glamorgan Council. Our team are based at Barry Hospital working across the Vale of Glamorgan and we operate all year round, 7 days a week.
Either a hospital, GP or health & social care professional will submit a referral on a patient's behalf requesting services from VCRS. Our team will work closely with other professionals regarding their therapy, health and social care support requirements and will feed back to them on your progress.
How can VCRS help?
VCRS will provide therapy, reablement care and support to patients at home rather than in hospital and support with rehabilitation to aid recovery. We will carry out a free period of assessment of service users' therapy/health and social care needs with them; taking into consideration what matters most to them, and together we will develop a rehabilitation programme to maximise their independence.
We will discuss what service you can expect us to deliver in your home. We cannot guarantee an exact time for our services; however, we can give an approximate time. We will continue to monitor progress, aiming to reduce the levels of support as your physical health and well-being improves.

The Reablement social care team operates everyday working evenings and weekends (including Bank Holidays) from 07.00am – 22.30pm to provide care and support calls to meet the needs of the individuals and the requirements of the service.
VCRS aim to rehabilitate people in their own homes to prevent hospital admissions and to re-able people who have returned home following a hospital stay whilst any future care and support needs are determined.
Reablement is a strengths-based, person centred approach within Health and Social Care that aids people to learn or re-learn skills necessary to be able to engage in activities that are important to them that promote and maximises independence and wellbeing.
